What changed
Add the public issue form used by FTW's Report driver issue action.
The form asks for the short sanitized FTW diagnostics, reproduction steps and expected result. It blocks submission until the reporter confirms that IP addresses, serial numbers, credentials, site IDs, raw config, logs and private documents were removed.
