fix: commit tool outputs before post-tool replies

[chenghao-mou]

A completed tool result stayed outside the canonical chat context until its follow-up speech finished. A late EOU could therefore accept and play a preemptive reply generated against the in-progress placeholder.

Port the eager commit ordering from Python PR #6349. The same late EOU now invalidates the stale generation and regenerates from the real result.

The regression covers placeholder preemption, post-tool speech interruption, and context-aware regeneration.
