Document the stranded-machine outage and its fix - #147

Conversation
On 2026-10-29 Fly migrated the app machine onto a host that then had no shared CPU left, and the proxy retried that host forever. Record the log signature and the clone-then-destroy fix beside the one-machine rule it has to respect, and say why one machine is kept regardless. A runbook paragraph in
docs/design/deploy.md, under the one-machine section, for the outage of 2026-10-29: Fly migrated the app machine onto a host that then ran out of shared CPU, and the proxy kept retrying that host. It records the signature (stopped machine,start/proxythencrash/flyd exit_code=0loop, no app log lines, "insufficient CPUs" from the proxy) and the fix (fly machine clone, then destroy the old one), and says why one machine is kept anyway.Docs only, no code change.
