test: establish recognition challenge v2

Summary

• replace the 24-case challenge with a 48-case independently authored v2 matrix
• add twelve positive/refusal semantic boundary pairs and fixture-leakage checks
• add a deterministic legacy semantic-path audit with explicit issue ownership
• record the honest 41/48 hard-cutover baseline and correct stale protocol documentation
