2FA / IdP login issue, and add manage devices to theme

Justin observed an issue recently when already connected to an authenticator app/device, and resetting password, he also had to recreate the device to receive OTP codes. So in theory, he then had 2 devices registered.

1. Need to test and see if we can reproduce, and figure out how to fix this login flow.
2. We should add a link near the reset password to manage connected OTP devices and remove old devices.
3. When multiple OTP devices, the radio labels don't align with the labels.
