feat: 변경 요청을 창 하나로 모으고 추가 포트 변경을 요청할 수 있게 함 - #214

기간 연장과 그룹 추가가 각자 모달과 제출 함수를 따로 갖고 있어, 종류가 늘 때마다 같은 코드가 복사됐다. - ChangeRequestModal: 종류를 고르면 폼만 바뀌고 사유·제출은 공통이다. - changeRequestTypes: 종류마다 폼·처음 값·검사·보낼 값을 한 곳에 등록한다. 기간 연장, 공유 그룹 추가, 추가 포트 변경(새로 추가). - 이미 검토 중인 종류는 선택지에 표시하고 제출을 막는다. - 관리자 화면: 포트 변경 승인을 막아 두던 설정을 풀고, 승인하면 반영 중으로 표시한다. 접속 안내는 용도 이름에 vnc가 들어 있으면 원격 데스크톱 주소로 안내하는데, 폼 검사는 예약된 이름과 정확히 같을 때만 막아 tigervnc 같은 이름이 통과했다. 원격 데스크톱은 켜지지 않으므로 틀린 안내가 나온다. 새로 추가하는 포트의 용도 검사를 한 함수로 모으고 vnc가 들어간 이름도 거른다. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
