Expected:
the unit to be at normal strength

it is in crusade mode for tyrannic war and is attached to a squad of BGVs

The Apothecary in the attached list is not attached to a unit of BGVs and is not fatigued. Unable to reproduce the issue.

This is an intended behaviour: the reason the Apothecary in your list has its OC and stats altered while attached to the Bladeguard Veterans is that the Bladeguard are currently under the effect of the Fatigued Battle Scar. Because that Battle Scar applies to "this unit", attached characters are also affected while assigned to the squad.
